Weekly therapy can be an effective source of support for many mental health and substance use concerns. But needs can change. Symptoms may become harder to manage, daily functioning may decline, substance use may increase or a person may need more structure than one appointment each week can provide.

Needing a higher level of care does not mean therapy has failed. It may simply mean that the frequency, structure or type of support needs to change.

Depression, anxiety, panic, trauma-related symptoms, mood changes or other mental health concerns may become more frequent, intense or difficult to manage between appointments.

3. Substance Use or Relapse Is Increasing

Repeated return to alcohol or drug use, escalating use or difficulty applying relapse-prevention strategies may indicate that more frequent treatment and support should be considered.

4. You Are Struggling to Stay Safe

Thoughts of suicide, self-harm or immediate danger require prompt attention. If there is an immediate risk of harm, call 911, go to the nearest emergency department or contact the 988 Suicide & Crisis Lifeline. A routine PHP or IOP assessment should not replace emergency evaluation when immediate safety is at risk.

5. Alcohol or Drugs Have Become a Primary Coping Strategy
